About NDPAP

Built by post-acute operators, for the providers and families they serve.

NDPAP is the National Directory of Post-Acute Providers — a free, independent directory of hospice, home health, DME, and pharmacy providers across the United States. We exist for one reason: small and mid-sized providers were losing visibility to platforms owned by national players, and families couldn't find them.

Why we built this

The biggest provider in the city shouldn't be the only one a family can find.

Our team has run hospices and home health agencies. We know what a 4pm discharge call sounds like, what a survey deficiency feels like, and what it takes to fill a census month after month with patients who actually fit your program.

We also know what happened to digital visibility for post-acute providers over the last decade. Directory after directory got rolled up. Most of what's left is owned by lead-broker companies that sell families' contact information back to providers — per click, per call, per admit. That model fights against the small operator who can't outbid a national chain, and it puts a tax on every family already navigating a hospital discharge.

NDPAP is the directory we wanted when we were operators. It surfaces every Medicare-certified provider in the country at no cost to families, and it gives small and mid-sized providers a place to stand on equal footing with the chains in their own cities.

What we won't do

The model is the message.

Every directory has rules they live by. Most are unstated. Ours are these.

AKS-clean by design

Sponsored listings are flat-fee advertising. Never per-patient, never per-referral, never tied to clinical steering. Compliant with the federal Anti-Kickback Statute by structure, not by promise.

Family-first defaults

No lead forms, no email gates, no account required to search. Every listing has the provider's real phone number, address, and services. A family in a hospital hallway can call in three taps.

Operator-respectful economics

A small hospice or home health agency pays the same flat rate as a regional chain to be Premium in the same city. No bidding wars. No "buy more impressions to stay visible." Three Premium slots per city, per provider type, period.

How NDPAP makes money

Premium listings. Flat fee. That's it.

Providers can claim their listing for free, forever. Anyone who claims gets a verified badge, full control of their profile, and inclusion in organic search results.

Premium listings cost $99 per month or $995 per year. Premium providers appear above organic results when families and case managers search their city. Three Premium slots are available per city per provider type, on a first-come basis. Premium status is purchased outright at a flat rate. It is not auctioned, it does not depend on family contact volume, and it cannot be tilted by clinical decisions.

Families never pay for anything on NDPAP. There is no tier where family search is gated, slowed, or sold. The directory is free at the point of use and always will be.

For the people we built this for

If you're a family, a discharge planner, or a case manager: this is the directory we wished existed when we were on your side of the call.

Search by city, ZIP, or provider type. Filter by what actually decides a referral — accepting new patients today, weekend coverage, languages spoken, insurances accepted. Tap the phone number and call. No account, no email, no broker.

For providers

Run a hospice, home health agency, DME supply, or specialty pharmacy?

Your listing is probably already in the directory. Claim it free, take control of the contact info families see, and decide whether you want a Premium slot in your city.
